These items are the paper forms that accompanied the physical toys. Included in this lot are: 4 Red & white Doctor appointment forms; 6 Visiting nurse appointment forms; an Eye Chart; 2 warning plaques; 3 certificates of health (2 different sizes and colors); a sick chart from Tinytown hospital, a prescription form; and rules for the visiting nurse. 19 items altogether.
